A Letter to My Patients

December 1, 1999

Dear Patients,

I am pleased to announce that as of January 1, 1999, Dr. Adam W. Brazus and I are joining in practice.  For some time I have been seeking a physician for my practice to provide a continuity of care while I take more time off for personal endeavors.  It has been a difficult task since I was looking for a physician who I could be assured would provide the complete, conservative and quality of care I felt my patients deserve.  Dr. Brazus fulfills all these criteria and when you meet him I am sure you will agree.  He is a well-trained and experienced Ophthalmologist and is a great addition to the practice.  You will find enclosed a brief biography as a way of introducing Dr. Brazus to you.

For any appointments you may have in the future, you may be seen by Dr. Brazus or myself, depending on who is available and in the office at that time.  You will continue to be received and cared for my the excellent staff with whom you are all already familiar .  With Dr. Brazus here we will be able to continue to be of service to you for years to come and now on a more timely basis.

Dr. Brazus and I look forward to continuing to care for your eye health needs and appreciate your allowing us to do so.  If you have any questions regarding this information, please don't hesitate to call the office.  Either the staff, Dr. Brazus or myself would be happy to talk with you.  We can assure you that you will continue to receive the excellent eye care you deserve.

Sincerely,

James J. McCallum, M.D.
